Welcome to All of Us Pennsylvania Online Scheduling

We are creating the largest health data resource ever. Your health information will be added to the All of Us Research Program database. Researchers may use this data in health studies. This may lead to the next medical breakthrough.

Please review the following study enrollment steps and then click Continue when you are ready to schedule an appointment.

Consent

You will be presented with two study consents. You can complete these online or at one of our enrollment centers.

  1. Basic Consent (15 to 20 minutes)

    If you decide to join the All of Us Research Program, you will need to enroll and give your consent.

  2. Electronic Medical Consent (10 to 15 minutes)

    We will ask for your permission to include your electronic health record (EHR) in the program. If you agree, researchers will have information about the health care you have received in the past, as well as the care you continue to get from your doctor or other health care providers. This will give researchers even more insights that will enable them to study patterns in the data. This will help them identify why people respond to treatments differently.

Surveys

Once you have enrolled and given your consent, you will be asked to answer health-related questions. Your answers will help to build the All of Us database. Researchers can access this database to conduct their studies.

Surveys include:

  1. Basic Information (10 to 15 minutes)

  2. Overall Health (15 to 20 minutes)

  3. Lifestyle (15 to 20 minutes)

Physical Measurements & Biosample Collection

During your visit, you will provide blood and urine samples and we will take your physical measurements.

  1. Physical Measurements (10 minutes)

    We will obtain your height, weight, blood pressure, heart rate, hip and waist circumferences.

  2. Biosamples (15 to 20 minutes)

    We will collect blood and urine samples. There is no need to fast prior to your appointment.
