To get there, we are creating the largest health data resource ever. The health data you share will be added to the All of Us Research Program database that researchers can access and conduct studies. This information is essential to creating a healthier future for generations to come and may lead to the next medical breakthrough.

Here is an overview of the steps involved in joining the program.

Provide an email address or mobile phone number. You will also need to create a password.
| Give Your Consent |
|
You will be presented with two study consents. You can complete these online or at one of our enrollment centers.

Basic Consent (15 to 20 minutes)
If you decide to join the All of Us Research Program, you will need to enroll and give your consent.

Electronic Medical Consent (10 to 15 minutes)
We will ask for your permission to include your electronic health record (EHR) in the program. If you agree, researchers will have information about the health care you have received in the past, as well as the care you continue to get from your doctor or other health care providers. This will give researchers even more insights that will enable them to study patterns in the data. This will help them identify why people respond to treatments differently.

Once you have enrolled and given your consent, answer the health-related questions, which will help to build the All of Us database. Researchers can access this database to conduct their studies.

Schedule your appointment online or call 800-664-0480.. We will need to collect some basic contact information from you. You will answer a few questions to see if this study is right for you. If you decide to enroll, you may pick the location, date, and time that works best for you to complete your one-time visit.

During your visit, you will provide blood and urine samples and we will take your physical measurements.

We will obtain your height, weight, blood pressure, heart rate, hip and waist circumferences.

Biosamples (15 to 20 minutes)
We will collect blood and urine samples. There is no need to fast prior to your appointment.
